Rental Form This includes the rental policy and the agreement form. To initiate a rental of a Saltillo product, complete the rental agreement and return to Saltillo.

Insurance

Notice of Privacy This describes the Saltillo Corporation's Privacy Policy when handling billings to insurance.

Client Information (pdf format) This form needs completed if Saltillo will be asked to bill insurance.  (Saltillo will only bill insurance for products manufactured by Saltillo)
